What Dental Issues Can Invisalign® Fix?

invisilign clear braces cosmetic dentist boulder colorado When you have spacing, alignment, or bite issues with your teeth, you may feel embarrassed to show your smile. What’s more, the long-standing and traditional means for addressing these aesthetic concerns have been metal braces, which many find equally embarrassing and uncomfortable. Thankfully, however, there have been recent advancements in braces technology with Invisalign® Clear Braces leading the way.

Most people find traditional metal braces a nuisance in look and feel. The process of straightening teeth with this method often takes years and can be painful during adjustments. Since Invisalign® Clear Braces have come onto the market they’ve revolutionized the way people feel about having to need braces. That’s because they offer many benefits over traditional braces, most importantly, offering an aesthetically pleasing alternative to a mouth full of metal.

While the predominant motivation for braces is to fix how your teeth look, it also aids in the overall health of your mouth, as these cosmetic issues can lead to other oral health concerns in the future. At Adler Advanced Dentistry, Dr. Michael Adler understands this and works with you to make sure that your teeth will not only look great but will be healthy as well.

Invisalign® Clear Braces can help with a number of cosmetic dental concerns, including:

  • Overbite: This happens when your upper teeth hang over your lower teeth. This can lead to problems with your gums, excess wear on your lower teeth, and jaw and joint issues if left untreated.
  • Underbite: This is when the lower teeth stick out past the top teeth. This may also lead to jaw and joint issues as well as excess tooth wear.
  • Gaps in teeth: This happens if your jawbone continues to grow more than normal or if you have a missing tooth, surrounding teeth may shift and create gaps. This can lead to problems with your gums and heightened risk of periodontal disease if left untreated.
  • Crossbite: This occurs when the top and lower jaws are misaligned. This issue can create wear on the teeth, bone deterioration, and increased risk of gum disease.
  • Open bite: This issue is when some teeth cannot make contact with opposing teeth to create a correct bite. This can lead to painful chewing, speech issues, and even risk of developing TMJ.
  • Crowded teeth: This occurs when there is not enough jaw space for all the teeth to fit normally. Left untreated, teeth can become excessively crooked, have increased plaque buildup, tooth decay, and an increased risk of gum disease.

As you can see, these issues, while being aesthetically unpleasing, can also lead to serious oral health concerns when left untreated. Invisalign® Clear Braces can help fix these dental issues and give you the confidence that your smile isn’t marred by metal.

Dr. Alder will go over all of the details about Invisalign® Clear Braces during your consultation and also determine your candidacy for this treatment. New Sleep appliance for snoring and Sleep apnea

We just started using a new sleep appliance called MicroO2 with great success. This appliance seems to be much more comfortable than the Somnomed appliance. We have stopped significant snoring and improved oxygenation almost immediately, even in very severe cases. The appliance also does not constrict the tongue space- a common problem with other sleep appliances. When we call our patients after a few days of wear they are amazed with the results. We use a neuromuscular bite as the starting point for the appliance. This allows a much more comfortable jaw position. There is a greater than 70% overlap between TMD and sleep apnea so by combining neuromuscular dentistry techniques into the design of the appliance makes sense. We begin to treat both problems at the same time and find this to be integral to success. I will post some pictures and some sleep studies showing how the appliance works.

2011 Full Mouth Winning Case from IACA Competition.

This female patient first came to us with some very strong esthetic concerns. She was very unhappy with her own smile and overall dental health. The owner of her own advertising firm, she was often “in the public eye,” so a beautiful smile was a key aspect of her working life. Having seen a variety of the past patient portraits we had published, she was very interested in how we could give her the smile she wanted.

We started off, as we always do, with a discussion of her wants and desires and a full clinical exam. During that examination, I realized very quickly that I would be unraveling a tangled mess. She was missing some molars, one posterior crown (#30) had fallen off, there were caries evident, she had some facial asymmetry and she was experiencing neuromuscular symptoms with definite ‘clicking and popping’ in her joint along with TMJ pain. Her canine (#6) was actually between #4 and #5 (as you can see in the Before photos) with its roots tangled in amongst the roots of those teeth.

Following the clinical examination, we suggested a three- phase treatment plan: the first phase was to stabilize her dental situation so we could eventually properly evaluate her bite. We started of f by placing a permanent crown on tooth #30,  replacing the one that had fallen off and restored the caries evident throughout. Then we had to extract tooth #6 given its location and clinical situation. She was absolutely opposed to orthodontic treatment, stating “No braces” whenever it was suggested. While this hampered our efforts to fully correct her mid-line in the final result, we were able to get her to wear a Removable Expansion appliance on her upper arch (although her compliance was not the greatest). The patient finally did agree to the use of Invisalign to correct her anterior crossbite. This was critical in avoiding the extensive preparation that would have been required otherwise.

Once all of this was accomplished, we were able to take the K-7 and TENS devices to determine her comfortable bite position. A Lower Fixed LVI Orthotic, fabricated from a Myobite, was placed to correct the patient’s jaw alignment and to calibrate proper ver tical positioning. The orthotic was placed and remained in position for 5 months. At that point, we checked the patient’s revised bite on the K7. Both the patient and I were satisfied with the new bite, everything was in alignment, the symptoms had disappeared and she was comfortable.

Aurum Ceramic/Classic prepared a Diagnostic Wax-up as per their ACCES™ system, showing the patient her new smile. She was insistent on a very light shade, tracing back once again to her strong dissatisfaction with the shade of her original natural smile. Her upper arch was prepared first and finished following the recommended LVI protocol for esthetic reconstruction with the Aurum Ceramic/Classic Advanced Esthetic (AE) Team providing the Prep Indices, Bite Stent and Siltec Provisional Stent. All measurements were carefully maintained as we proceeded to restore the case.

All 28 teeth were restored at one appointment. The upper arch was restored with IPS e.max®  crowns and IPS Empress® veneers, all beautifully crafted by Aurum Ceramic/Classic. We actually chose to move the #5 bicuspid to the canine position, changing the shape of the restoration to improve the overall appearance of her smile. Her lower arch was restored with IPS e.max crowns and a Contessa™ Zirconia bridge #28 – #30 (to replace her missing #29). Aurum Ceramic/Classic matched the shades beautifully, you really can’t tell which is IPS e.max and which is porcelain.

The patient has been extremely happy with the final result both neuromuscularly and esthetically. In fact, she has sent us a veritable torrent of gifts for the office. We must  have received 20 to 30 boxes of different types of foodstuffs since we placed the case.

Invisalign Special | Boulder Denver Adler Cosmetic Family Dentistry Announces $500 Off Invisalign Treatment

Nationally recognized Colorado based cosmetic dentist Dr. Michael Adler announces Invisalign invisible braces promotion effective for June 2010.

Boulder, Colorado, June 3, 2010 –  Adler Cosmetic & Family Dentistry today announced a special limited time discount for Invisalign teeth straightening treatments begun in June 2010.

To celebrate the start of summer Dr. Michael Adler of Adler Cosmetic & Family Dentistry has designated the entire month of June to be ‘Invisalign Month’. The first five patients that begin Invisalign treatment during the month of June will save $500 on the cost of their treatment. Patients that are not one of the first five to take advantage of this special offer will still save $300 on the cost of Invisalign treatment begun in June. The special Invisalign pricing is intended to provide patients with an added incentive to get that great looking smile they’ve always wanted while still saving summer fun money.

Additionally Dr. Adler is providing free consultations to answer questions about Invisalign treatment. Patients who’ve wondered whether Invisalign is the right treatment for them are encouraged to take advantage of this special opportunity.

Invisalign is the virtually invisible way to straighten teeth without braces. The Invisalign system uses a series of clear, removable aligners to move teeth over time to give the smile patients want and deserve.

Among the chief advantages of using Invisalign are:

  • Absence of ugly, metal bands and brackets
  • Results in months instead of years
  • Aligners are virtually invisible, so it’s nearly imperceptible to others that you’re undergoing orthodontic treatment.

Among the conditions that are treatable using Invisalign are: 1) Overly crowded teeth. This occurs when there is simply a lack of room within your jaw for all of your teeth to fit normally, 2) Widely spaced teeth. This occurs with abnormal continued growth of the jaw bone, 3) Crossbite. This occurs when the upper and lower jaws are both misaligned, 4) Overbite. It occurs when the upper teeth bite over the lower teeth, and 5) Underbite. This occurs when the lower teeth protrude past the front teeth.

Straighter teeth not only look better but are better for you also. Straighter teeth are easier to clean and so less prone to get cavities. I did Invisalign about 4 years ago. I had an overbite, a gap in my front teeth, and a tooth on the low that was rotated. I had wanted to have my teeth straightened for a while but when I got engaged I decided that it was time to have the picture perfect smile I had always wanted. Prior to the start of my Invisalign treatment I had all my cavities filled. I needed more fillings then I care to admit. During my first appointment, impressions were taken of my teeth. This was easier said then done since I had a gag reflex that was active that day.

The impressions were sent to Invisalign where they created a computer generated model of my teeth, known as a ClinCheck, showing how each aligner was going to move my teeth. This Clincheck was shown to me so the doctor and I could make any changes to the treatment plan before the aligner were actually fabricated. It was fascinating to see how my teeth would be transformed during treatment to the final results. Once the ClinCheck was approved by the doctor, Invisalign fabricated all of my aligners. It was important for me to have my fillings done before treatment started because all the aligners are created at the beginning and any changes to my teeth could have resulted in the aligners not fitting properly.

My Perfect Smile for My Perfect Day

Once the aligners arrived, clear “buttons” were bonded to selected teeth. “Buttons” are small dots of composite resin that help the aligners to move selected teeth into the proper position. IPR (inter proximal reduction) was also done at this and other appointments. IPR is where the dentist removes a very minuscule amount from between certain teeth to allow them the room to move properly. Placing the buttons and doing the IPR was quick and painless. I wore my aligners 24/7 except to take them out to brush and eat. It was great. No one knew I was wearing them and I could still eat and clean my teeth like normal. I would change the aligners every 2-3 weeks. They would feel tight at first but by the time it was time to change them, they felt loose and comfortable. I got to the point where it felt strange not to have my aligners in. I could even see the changes in my teeth throughout treatment.

I was engaged for a year and also did Invisalign for a year. Once treatment was over, the buttons were removed and I only had to wear my retainers at night to keep my teeth from shifting back. The retainers are like traditional retainers. They are exactly like the last aligners but don’t have the button attachment bumps and are of a sturdier material. It was easy and pain free. I was done with treatment in time for my wedding and I had the picture perfect smile that I always wanted just in time for my perfect day.

If you want to see my results just ask for Kim, I work as both front office and dental assistant for Dr. Adler at Adler Cosmetic & Family Dentistry.

Invisalign | Clear Braces Revolutionizing Dentistry

Boulder Dentist Serving Denver, Ft. Collins & Nearby Areas in Colorado

Dr. Adler has been an Invisalign provider for the past seven years.  He stays on top of Invisalign’s new developments and improvements by regularly attending continuing education courses, such as the one he is attending in March of 2010.  He is constantly striving to not only offer his patients the best products available but to also improve his own knowledge and skills to offer them the best treatment possible.

Invisalign has been one of those revolutionary products in dentistry that has greatly contributed to helping patients improve their overall dental health and enhance their smiles both cosmetically and functionally without interrupting their lives.  Invisalign is a great alternative to traditional braces to straighten teeth.  With traditional braces patients have to wear metal brackets and wires for years in some cases that are uncomfortable, irritating the cheeks and gums, and are difficult to keep clean.  In addition to all this they are also unattractive.  Anyone who had traditional braces as a child will cringe at the memory of being called brace face or metal mouth.  Adults are even more hesitant to have traditional braces placed because they feel self-conscious and not taken seriously.  Invisalign allows you to straighten your teeth without looking like you are.  Invisalign uses series of clear plastic trays called aligners that are custom made to gradually move your teeth into place by switching out aligners about every two weeks.  They are easy to use and are removable to allow regular eating and brushing.

Invisalign is constantly striving to improve their products.  One of their latest advancements is their new Invisalign Teen.  This is a program that is tailored for teens.  The aligners have a Blue Dot Wear Indicator designed to show an estimation of wear-time.  For any lost or misplaced aligners, it comes with up to six free replacement aligners.  Invisalign is constantly expanding the difficultly and types of problems that can be treated.  Treatable conditions include overly crowded teeth, widely spaced teeth, crossbite, overbite, and underbite to name a few.  Invisalign is not just for improving teeth cosmetically but is an important tool for improving a patient’s overall health and wellness.  Straighter teeth are easier to clean and less prone to infection.  People will often think that infections in the mouth are somehow isolated from the rest of the body but this could not be further from the truth.  Dental infections can easily spread throughout the body.  Good dental health improves the health of the entire body.

Getting Aligned with Invisalign Invisible Braces

before-smile-sm1 Jeff came to Dr. Adler of Adler Cosmetic & Family Dentistry seeking to brighten and straighten his smile.

Dr. Adler sat down with Jeff and discussed exactly what Jeff’s expectations and goals were.  He found that Jeff wanted his teeth to be healthy but also wanted them too look good and appear natural.  Based on Dr. Adler’s thorough exam, he noted that Jeff had decay on several teeth and that one of his lateral teeth (#10 upper left next to middle front teeth) was peg shaped, under sized and out of proportion.  He also saw that Jeff’s teeth were discolored and varied in shade, crowded, in an unhealthy bite, and had extreme wear and gum line problems.  Dr. Adler recommended that Jeff first have fillings done to remove the decay and get his teeth healthy and then proceed with Invisalign and Zoom Whitening to straighten and brighten his teeth.  He also recommended that Jeff restore his lateral teeth (#7 and 10, on either side of his upper front teeth) to their correct size and proportion with porcelain veneers. The fillings were important to do first not only to remove the decay before the cavity got worse but also because Jeff would not have been able to have any restorative work done once the Invisalign treatment had started until it was finished.  This is because the trays that are used in the treatment are made all at once and progressively shift the teeth into position as the trays are changed out.  If restorative work was done during treatment then the trays would not fit.

Jeff Before

Jeff Before

For each tooth with a cavity, Dr. Adler removed the decay from the tooth and then placed tooth colored composite resin in the prepared tooth reshaping it to its original shape.  A harmless curing light is used to harden and set the material.  The tooth is then good as new and the filling is unnoticeable.

Once all the cavities had been restored with composite fillings, Dr. Adler was able to start the Invisalign.  An impression of Jeff’s teeth and bite were taken and then sent to Invisalign labs.  There a model of Jeff’s mouth was made and scanned as 3-D virtual image.  They then made small incremental movements of the virtual teeth to simulate how the aligners would shift the teeth into their final position.  Buttons, small tooth colored composite dots placed on selected teeth, were required to help certain teeth shift and rotate more.  Once this virtual model was completed, it was sent to Dr. Adler for approval and/or modification. Once approved, the aligners were fabricated for the entire treatment phase and sent back to Dr. Adler.  The buttons were places on the selected teeth and Jeff was given his first set of aligners.  Each aligner was worn for about two weeks and each new aligner was slightly different to help slowly move the teeth into there proper position.  The aligners were clear so it was hard for anyone to know that Jeff had “braces” on.  They were removable for easy cleaning and were not worn when eating.  Since Jeff had crowded teeth, IPR (inter proximal reduction) was required to give the teeth more room to move.  IPR is a very simple and painless procedure that involves Dr. Adler filing a very minuscule portion between the teeth to create a small amount of space that the teeth will later move into. Treatment was completed in 6 month, much less time than traditional braces.  The buttons were removed once treatment was done and clear aligner retainers were given to be worn at night to keep the teeth from shifting back.

Jeff had Zoom Whitening treatment done to brighten his new smile and remove the discoloration.  The after-smile-sm1 Zoom whitening gel was applied to each tooth and then cured with a high intensity curing light that activates the whitening gel.  The Zoom light aids in activating the hydrogen peroxide and helps it penetrate the surface of the tooth.  This procedure was repeated four times to maximize the whitening effect.  After the treatment, Jeff had a noticeably brighter smile.  He was given more whitening gel to use with his Invisalign retainers for any need touch up or maintenance bleaching.

Now that Jeff’s teeth were all a beautiful shade of white, the veneers could be placed.  A veneer is a thin shell of porcelain that is bonded to a tooth to improve its color or shape.  A veneer generally covers only the front and top of a tooth.  Veneers can be used to close spaces between teeth, lengthen small or misshapen teeth, or whiten stained or dark teeth.

Jeff After

Jeff’s two upper front teeth on either side of the middle teeth (#7 & 10) were first shaped and roughened to prepare them for the veneers.  Impressions were then taken of the teeth and sent to Aurum Ceramic Dental Laboratory, where the veneers were artistically custom crafted.  Once the veneers had returned from the lab, they were cemented onto Jeff’s teeth using a durable adhesive bond.  The bond was then cured with a harmless high-intensity light.  This final touch completed Jeff’s treatment and he now has exactly what he wanted, a straight and white natural looking smile.  The results are stunning and Jeff loves showing off his smile now.
