Straighter teeth not only look better but are better for you also. Straighter teeth are easier to clean and so less prone to get cavities. I did Invisalign about 4 years ago. I had an overbite, a gap in my front teeth, and a tooth on the low that was rotated. I had wanted to have my teeth straightened for a while but when I got engaged I decided that it was time to have the picture perfect smile I had always wanted. Prior to the start of my Invisalign treatment I had all my cavities filled. I needed more fillings then I care to admit. During my first appointment, impressions were taken of my teeth. This was easier said then done since I had a gag reflex that was active that day.
The impressions were sent to Invisalign where they created a computer generated model of my teeth, known as a ClinCheck, showing how each aligner was going to move my teeth. This Clincheck was shown to me so the doctor and I could make any changes to the treatment plan before the aligner were actually fabricated. It was fascinating to see how my teeth would be transformed during treatment to the final results. Once the ClinCheck was approved by the doctor, Invisalign fabricated all of my aligners. It was important for me to have my fillings done before treatment started because all the aligners are created at the beginning and any changes to my teeth could have resulted in the aligners not fitting properly.
Once the aligners arrived, clear “buttons” were bonded to selected teeth. “Buttons” are small dots of composite resin that help the aligners to move selected teeth into the proper position. IPR (inter proximal reduction) was also done at this and other appointments. IPR is where the dentist removes a very minuscule amount from between certain teeth to allow them the room to move properly. Placing the buttons and doing the IPR was quick and painless. I wore my aligners 24/7 except to take them out to brush and eat. It was great. No one knew I was wearing them and I could still eat and clean my teeth like normal. I would change the aligners every 2-3 weeks. They would feel tight at first but by the time it was time to change them, they felt loose and comfortable. I got to the point where it felt strange not to have my aligners in. I could even see the changes in my teeth throughout treatment.
I was engaged for a year and also did Invisalign for a year. Once treatment was over, the buttons were removed and I only had to wear my retainers at night to keep my teeth from shifting back. The retainers are like traditional retainers. They are exactly like the last aligners but don’t have the button attachment bumps and are of a sturdier material. It was easy and pain free. I was done with treatment in time for my wedding and I had the picture perfect smile that I always wanted just in time for my perfect day.

Boulder Kid Dentist | Happy Visit | Great First Experience for Children
We have learned from our own experiences as well as from our patient’s, that traumatic experiences in the dental office happened as children. We commonly hear that the dentist was very short and impatient, and they experienced pain and trauma during the procedure. Many of these patients may not have had a bad experience since, but are still nervous or even scared of going to the dentist because of their first experience.
Dr. Adler understands a childs first visit to the dentist is important. At Adler Cosmetic & Family Dentistry, we like to start our newest patients off on the right foot with a “Happy Visit”. Children usually have their first visit to the dentist when they are about 2 or 3 years old. A Happy Visit should be exactly what it says – a happy visit to the dentist.
The child is welcomed into the office by our very patient and sweet hygienist, Susan. Susan recognizes that sometimes just showing the child our office is enough for the first time With a parent present, Susan gives the child a ride up and down in the chair to get used to the idea that it moves. Susan also plays a short video, “Andy’s First Visit”. With the help of two of the cutest Jack Russell terriers, the child is able to see the equipment and tools used by Susan in this dramatized version of dogs going to the dental office. Andy and Buddy is a gentle way to expose children to their first visit to a dental office. Susan will then interact with the child, such as letting them feel the suction, air and water. She talks to them about how she would like to look in their mouth and count there teeth and even brush all the “germ bugs” off of them so they are nice and clean. Dr. Adler also joins the fun and says hello. Dr. Adler does as much of an exam as the child is comfortable with. In some instances bitewing xrays are taken. Some children are not ready to have anything done , & are not pressured to do more than they are comfortable with. We want the overall experience to be positive. Kids always enjoy ending any visit to the office by picking a prize out of our Treasure Chest.
We have had several mothers tell us that their children were actually upset when they found out that mom went to the dentist without them. We love to see children come in who are excited for their appoinment and ask to be the one to go first! Through their patient and friendly methods, Dr. Adler and Susan have been very successful with their treatment of children and are grateful that the children enjoy coming to the dentist.
Invisalign | Clear Braces Revolutionizing Dentistry
Dr. Adler has been an Invisalign provider for the past seven years. He stays on top of Invisalign’s new developments and improvements by regularly attending continuing education courses, such as the one he is attending in March of 2010. Invisalign has been one of those revolutionary products in dentistry that has greatly contributed to helping patients improve their overall dental health and enhance their smiles both cosmetically and functionally without interrupting their lives. The aligners have a Blue Dot Wear Indicator designed to show an estimation of wear-time. For any lost or misplaced aligners, it comes with up to six free replacement aligners. Decay Under Crown | Patient Profile

- Tooth #2 with PFM and Access Filling
Dee had just moved to town and was due for her cleaning. She wanted to make sure that there were no problems with her teeth. She chose to come to Adler Cosmetic & Family Dentistry at her daughter’s recommendation, who was one of Dr. Adler’s long time patients.
During Dr. Adler’s comprehensive exam using an interoral camera, he noted that tooth #2, her upper right molar, had a porcelain fused to metal crown, also known as a PFM, that was breaking down. A composite filling in the top of the crown sealing an access opening showed that the tooth had had a root canal done after the crown had been placed. The root canal was apparent in the x-ray but Dr. Adler also saw that there was decay on the distal or back side of the tooth under the crown. He recommended that the existing crown be removed so the decay could be cleaned out and a new all porcelain crown be placed.
Once the crown was removed, Dr. Adler saw that the decay was more extensive. The patient said that she had pain on the tooth which had originally prompted the placement of the PFM crown. When pain had continued later after the crown was done, an access opening was drilled though the crown and a root canal was performed.

Root canalled teeth can sometimes be tricky to extract since the tooth is more fragile and likely to break a part during the procedure. Dr. Adler was able to successfully extract Dee’s tooth with no problems. Sometimes a tooth can not be saved despite a doctor’s best efforts if the decay has gone too deep. Dee is glad that the tooth no longer bothers her and that the decay was stopped in its tracts. She is now considering replacing the tooth by placing an implant, not only to replace the tooth but also to help keep the bone from receding. The implant acts like the roots of a natural tooth by supporting it and giving the bone something to attach to. Based upon Dee’s most recent experience she is making double sure that all of her old crowns are in good condition and don’t need to be replaced.

Whiter Teeth and Veneers in Time for Wedding

Jonathan came to Adler Cosmetic & Family Dentistry at his fiancé’s recommendation. He wanted to make sure his teeth were going to look good in time for his wedding. This was a major concern for Jonathan because two of his front teeth were dark and discolored from previous root canals and he didn’t like the spacing or color of his other teeth.
Dr. Adler addressed Jonathan’s concerns by offering him several treatment options. One option was Zoom whitening to brighten the color of his other teeth, Invisalign to straighten all of his teeth, a post for #8 (his upper front right tooth) for support and veneers for his front teeth since teeth darkened because of root canals would not lighten to the extent of his other teeth with just Zoom alone. A second option was veneers on the front four teeth with a post in #8 and the Zoom whitening with no Invisalign. This second option would fix spacing issues only in the front four teeth but not any of the other teeth. Jonathan chose the second option in order to have the treatment completed in time for his wedding and will consider doing Invisalign in the future.
Jonathan started the Zoom whitening after having a thorough cleaning to insure healthy gums and a good result. It is important to have a cleaning within 6 months prior to whitening to insure the best results. Whitening dirty teeth is about as effective as painting a dirty car. During the Zoom whitening, Jonathan’s gums and soft tissue are protected by a liquid dam. The whitening solution was then applied to the teeth and the activating light was used. The process was repeated in four consecutive sessions. The whole process took approximately two hours. Customized whitening trays were made during the appointment to allow for any needed touch ups at home. Jonathan was at least two shades lighter by the end of the appointment.

In order to fabricate the veneers and post, impressions were taken of Jonathan’s teeth and set to the laboratory so they could create a wax-up. A wax-up is a wax model of what Jonathan’s veneers will look like on actual models of his teeth. This was used to insure that the look and size of the veneer would be correct to give Jonathan the most aesthetic smile. Once approved, Dr. Adler prepared the four upper front teeth for the veneers and post. Veneers require very little of the natural tooth to be removed in order prepare the teeth. The post on #8 was needed to give added support since that tooth had been root canalled previously. Temporaries are then placed on the teeth until the final crowns are ready. Even with just the temporaries on, Jonathan’s teeth were already looking good.
When fabricating any teeth, especially front teeth, Dr. Adler and the lab make sure that they have the best color match possible to give the patient a bright but natural smile. The aim is to have great looking teeth but not be able to pick out which ones are real and which ones are not. Careful consideration was taken to make sure that Jonathan’s teeth were both well shaped and shaded. Once the custom fabrication was completed, Jonathan returned to have the final restorations cemented. The temporary veneers where removed and a high quality translucent cement was used to chemically bond the veneers and post to the teeth. The results were dramatic.

Dr. Michael Adler of Boulder, Colorado recently attended the 15th International Congress of the International College of Cranio-Mandibular Orthopedics (ICCMO) in Fukuoka, Japan on November 14-15, 2009.
Dr. Adler was one of fourteen members of ICCMO to attain the distinguished credential of Fellowship during the meeting. In a formal, robed Convocation, Fellowship was conferred upon Dr. Adler who successfully fulfilled rigorous obligations as required by the College. The Fellowship process has a minimum one year requirement. The major objectives of the Fellowship Program of ICCMO are to provide recognition of an individual for a high level of achievement in the diagnosis and treatment of craniomandibular disorders and for professional efforts to advance the science of the biophysics and physiology of the head and neck.
ICCMO is a society of health care professionals with a common interest in the etiologic role of orthopedic deformities as related to the various symptoms of head and neck pain and dysfunction.
